Do I need a CRM for my small business?

The trigger is not size — it is when more than one person needs the same information at the same time, because that is the point at which versions start disagreeing.

Should I buy off-the-shelf software or build custom?

Buy first, always. Off-the-shelf is cheaper, supported, and updated by someone else. Custom is worth it only where your process is genuinely different from everyone else in your industry and that difference is why customers choose you.
